Even in successful cases, however, the potential benefits of decentralization have not been fully realized. In some cases decentralization has even had detrimental effects and has contributed to the weakening of health systems (Lieberman and others, 2005). The transfer of managerial responsibilities to lower administrative levels has not always been accompanied by a parallel level of devolution in financial resources and political authority. Many local governments lack the necessary capacity to plan and manage local health services.

Measles remains relatively common in parts of the WHO Western Pacific region, where approximately 1 million cases of measles were estimated in 2003, 30,000 of them fatal (WHO, 2005h). The fact that the proportion of children vaccinated against measles in East and North-East Asia and the Pacific declined by 14 per cent and 30 per cent respectively between 1990 and 2003 is particularly worrisome. These two subregions are the only ones in the world where immunization coverage for these diseases has diminished (United Nations, 2005).
